What you’ll need
- Business legal name, DBA, address and phone
- Federal Tax ID (EIN), or owner details if you’re a sole proprietor
- Owner name, home address, date of birth and Social Security number
- Business bank routing and account number
- Estimated monthly card sales and average ticket
